Insurance Claims
If your phone has physical damage*, liquid damage, or was lost or stolen and you
are enrolled in Total Equipment Protection (TEP) or Equipment Replacement Program
(ERP) you may expedite the claim process by visiting www.phoneclaim.com to submit your claim request for
a replacement phone.
* Physical damage includes, but not limited to, items such as cracked displays, broken
flips, and damaged charging ports.
